All registrations are non-refundable.   If a cancellation is requested before the early registration deadline, a credit for future programs, may be issued at the discretion of GolfU.   After the early registration deadline all registrations are considered final and non-refundable.

2026 Program waiver information

By registering online, I acknowledge that I have carefully read all the information and agree with the terms listed below.

In consideration of the right of the individual listed in this registration form to participate in the GolfU programs, I hereby, on behalf of myself and the child (for whom I am the legal guardian), I hereby release from liability, discharge, hold harmless, and relinquish and waive any liability of and covenant not to sue South Jersey Junior Tour, GolfU, GolfU Franchising and any and all franchises of GolfU Franchising , (“GolfU”) and each of their successors and assigns, all other related entities, and all employees, directors, officers, agents, representatives, customers, purchasers and independent contractors of such entities, and the respective GolfU host golf club organizations and each of those entities’ respective affiliates, subsidiaries, franchises, officers, employees, agents and representatives of each of those entities from all liability, claims, demands, losses or damages, on my account or on account of the child identified above for whom I am the legal guardian, caused or alleged to be caused in whole or in part by the negligence of the releases or otherwise for any physical or mental injury, aggravation or any pre-existing illness or condition, permanent disability or death, property damage or other loss that may be sustained by me or my child in traveling to and from, or participating in, the GolfU program.
I hereby give permission for the Participant, who is my child or ward, to participate in the events. Further, I hereby:
1. agree that prior to participating, I will inspect the facilities and equipment to be used, and if I believe anything is unsafe, I will immediately advise an official of such conditions(s) and refuse to participate until such conditions(s) is corrected;
2. agree that my acts, poses, words and performances at the Events will be in good taste and will not reflect negatively on the image, reputation or business of the Released Entities and agree to indemnify and save and hold harmless each of the Released Entities from any loss, liability, damage or cost they may incur due to my participation in or otherwise in connection with the Events, whether caused by the negligence of any Released Entity or otherwise;
3. grant full permission to each of the Released Entities, GolfU Franchising LLC, GolfU LLC and any and all franchises of GolfU Franchising LLC, with no obligation to compensate me, to use my name, voice, statements, image, likeness, and/or actions, in addition to any photographs, videotapes, motion pictures, recordings, and/or other record of my performances(s) in the Events in perpetuity worldwide in any manner and in any and all media, whether now known or unknown or hereafter devised (the “Materials”), and represent that I have the right to grant such permission;
4. understand and agree that (a) my refusal to accept any prize from any game, event or contest (each, a “Contest”) or other activity shall constitute a forfeiture of that prize and shall release the Released Parties from any and all obligations in connection with such prize; (b) I am responsible for and shall pay all state and federal or other taxes on any and all cash and/or prizes I win; (c) I will indemnify and hold the Released Parties harmless against any liability for any such taxes and agree that any of the Released Parties may deduct or require payment of any such tax before delivery of a prize; and (d) any travel undertaken by me in the connection with my acceptance or use of any prize shall be taken at my sole risk. FURTHERMORE, I FULLY ACKNOWLEDGE, APPRECIATE AND UNDERSTAND THE RISK THAT MY PARTICIPATION IN ANY CONTEST, ACCEPTANCE OF ANY PRIZES AND/OR GolfU USE OF THE MATERIALS COULD AFFECT ANY HIGH SCHOOL OR INTERCOLLEGIATE ATHLETIC ELIGIBILITY THAT I MAY HAVE, AND I FULLY ASSUME SUCH RISK AND RELEASE THE RELEASED PARTIES FROM ANY LIABILITY IN CONNECTION WITH ANY SUCH FAILURE TO BE ELIGIBLE FOR HIGH SCHOOL OR INTERCOLLEGIATE ATHLETIC COMPETITION;
5. expressly agree that (a) this RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as is permitted by state laws (which shall govern this RELEASE AND WAIVER OF LIABILITY AND INDEMNITY AGREEMENT) and that if any portion hereof is held invalid, it is agreed that the balance shall nonetheless continue in full legal force and effect and (b) for the purposes of any action or proceeding arising out of this RELEASE AND WAIVER OR LIABILITY AND INDEMNITY AGREEMENT, I expressly submit to the jurisdiction of all federal and state courts located in the State of NJ; and
6. Acknowledge that novel coronavirus (“COVID-19%) infections have been confirmed throughout the United States, including several cases in New Jersey. In accordance with the most recent guidance and protocols issued by the World Health Organization (WHO), the Centers for Disease Control and Prevention (CDC), for slowing the transmission of COVID-19, the undersigned hereby agrees, represents, and warrants that neither the undersigned nor such participating children shall visit or utilize the facilities, services, and programs of GolfU (other than any exclusively online services and programs) within 14 days after (i) returning from highly impacted areas subject to a CDC Level 3 Travel Health Notice, (ii) exposure to any person returning from areas subject to a CDC Level 3 Travel Health Notice, or (iii) exposure to any person who has a suspected or confirmed case of COVID-19. The CDC Travel Health Network is continuously updating this list and the undersigned agrees that they are aware of this list and the countries listed.
The undersigned agrees to check the CDC Travel Health Notices list (https://www.cdc.gov/coronavirus/2019-ncov/travelers/index.html ) prior to utilizing the facilities, services, and programs of GolfU, on a daily basis if necessary. The undersigned hereby agrees, represents, and warrants that neither the undersigned nor such participating children shall visit or utilize the facilities, services, and programs of the GolfU if he or she (i) experiences symptoms of COVID-19, including, without limitation, fever, cough or shortness of breath, or (i) has a suspected or diagnosed/confirmed case of COVID-19. The undersigned agrees to notify GolfU immediately if he or she believes that any of the foregoing access/use restrictions may apply.
GolfU has taken certain steps to implement recommended guidance and protocols issued by the Public Health Agencies for slowing the transmission of COVID-19, including, without limitation, the access/use restrictions set forth above. The undersigned acknowledges and agrees that GolfU may revise its procedures at any time based on updated recommended guidance and protocols issued by the Public Health Agencies and further agrees to comply with the GolfU revised procedures prior to utilizing the facilities, services, and programs of GolfU. The undersigned further acknowledges and agrees that, due to the nature of the facilities, services, and programs offered by GolfU, social distancing of 6 feet per person among children and their caregivers in a childcare setting is not always possible. The undersigned fully understands and appreciates
both the known and potential dangers of utilizing the facilities, services, and programs of GolfU and acknowledges that use thereof by the undersigned and/or such participating children may, despite GolfU’s reasonable efforts to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability, and/or death.
IN FURTHER CONSIDERATION OF BEING PERMITTED TO ENTER GOLFU FOR ANY PURPOSE INCLUDING, BUT NOT LIMITED TO, OBSERVATION OR USE OF FACILITIES OR EQUIPMENT, OR PARTICIPATION IN ANY ON-SITE OR OFF-SITE PROGRAM AFFILIATED WITH GOLFU, THE UNDERSIGNED HEREBY AGREES TO THE FOLLOWING:
THE UNDERSIGNED, ON HIS OR HER BEHALF AND ON BEHALF OF SUCH PARTICIPATING CHILDREN, HEREBY RELEASES, WAIVES, DISCHARGES AND COVENANTS NOT TO SUE GOLFU, its directors, officers, employees, volunteers and agents from all liability to the undersigned or such participating children and all personal representatives, assigns, heirs, and next of kin of the undersigned or such participating children for any loss or damage, and any claim or demands on account of any property damage or any injury to, or an illness or the death of, the undersigned or such participating children (or any person who may contract COVID-19, directly or indirectly, from the undersigned or such participating children) whether caused by the negligence, active or passive, of GOLFU or otherwise while the undersigned or such participating children are in, upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with GolfU. The undersigned expressly and knowingly waives all rights under California Civil Code Section 1542, which provides: “A general release does not extend to claims that the creditor or releasing party does not know or suspect to exist in his or her favor at the time of executing the release and that, if known by him or her, would have materially affected his or her settlement with the debtor or released party.”
THE UNDERSIGNED HEREBY AGREES TO INDEMNIFY AND SAVE AND HOLD HARMLESS GolfU, its directors, officers, employees, volunteers and agents , and each of them, from any loss, liability, damages or costs they may incur, whether caused by the negligence, active or passive, or otherwise while the undersigned or any participating child is in, upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with GolfU. The undersigned understands and agrees that GolfU is not required to provide insurance to cover the undersigned or such participating children in the event they suffer illness, injury, death, property loss, theft or damage of any sort upon, or about the premises or any facilities or equipment therein or participating in any program affiliated with GolfU.
The undersigned agrees and acknowledges that use of the GolfU facilities and services, and participation in the GolfU programs, may involve inherent danger and risk, including, without limitation, the risk of physical illness or injury, death or property damage. THE UNDERSIGNED HEREBY ASSUMES FULL RESPONSIBILITY FOR, AND RISK OF ILLNESS, BODILY INJURY, DEATH OR PROPERTY DAMAGE to the undersigned or such participating children due to negligence, active or passive, or otherwise while in, about or upon the premises of GolfU and/or while using the premises or any facilities or equipment thereon or participating in any program affiliated with GolfU. The undersigned acknowledges that any illness or injuries that the undersigned or such participating children contract or sustain may be compounded by negligent first aid or emergency response of the Releasees and waive any claim in respect thereof.
THE UNDERSIGNED further expressly agrees that the foregoing ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY, AND INDEMNITY AGREEMENT is intended to be as broad and inclusive as is permitted by the laws of the State of New Jersey and that if any portion thereof is held invalid, it is agreed that the balance shall, notwithstanding, continue in full legal force and effect.

I HAVE CAREFULLY READ AND VOLUNTARILY SIGN THIS ASSUMPTION OF RISK, RELEASE AND WAIVER OF LIABILITY, AND INDEMNITY AGREEMENT AND FURTHER AGREE THAT NO ORAL REPRESENTATIONS, STATEMENTS OR INDUCEMENT APART FROM THE FOREGOING WRITTEN AGREEMENT HAVE BEEN MADE. I AM AWARE THAT BY AGREEING TO THIS AGREEMENT I AM GIVING UP VALUABLE LEGAL RIGHTS, INCLUDING THE RIGHT TO RECOVER DAMAGES FROM GOLFU IN CASE OF ILLNESS, INJURY, DEATH OR PROPERTY LOSS OR DAMAGE, INCLUDING, FOR THE AVOIDANCE OF DOUBT AND WITHOUT LIMITATION, EXPOSURE TO COVID-19 AT ANY GOLFU FACILITY OR PROGRAM AND ANY ILLNESS, INJURY OR DEATH RESULTING THEREFROM. I UNDERSTAND THAT THIS DOCUMENT IS A PROMISE NOT TO SUE AND A RELEASE OF AND INDEMNIFICATION FOR ALL CLAIMS. IF SIGNING ON BEHALF OF MINOR: I ALSO UNDERSTAND THAT THIS AGREEMENT IS MADE ON BEHALF OF MY MINOR CHILD(REN) AND/OR LEGAL WARDS AND I REPRESENT AND WARRANT TO GOLFU THAT I HAVE FULL AUTHORITY TO SIGN THIS AGREEMENT ON BEHALF OF SUCH MINOR(S).

I acknowledge that the Released Entities are relying on the grant of rights contained herein. By registering online, I acknowledge that a) I have permission to act on behalf of the Player in this registration and b) I have carefully read all the information and agree with the terms listed above.
